Insurance

We are In-Network/contracted with Delta Dental Premier Plan. We accept all PPO Plans under ALL insurance umbrellas however, we are considered Out-of-Network. As a courtesy, we will submit all claims to your insurance company and collect your estimated amount as treatment is rendered.

In House Plan

For an annual fee of $475/year (to be paid with cash or check; if paid by credit card, a 3% fee applies) the following standard maintenance procedures will be covered at 100% and any additional restorative services are discounted at 20% (to be paid with cash or check; if paid by credit card, a 3% fee applied). Patients typically visit the dentist every 6 months for a cleaning, exam and x-rays.

? Routine Cleaning/Prophylaxis (twice per year)
? Periodic Exam (twice per year)
? Required x-rays (once per year)
? Fluoride (once per year)
